Please list all children of your household who have your permission to be transported by church staff and volunteers for church events. Please note that the church van, rental vehicles, and personal vehicles all may be used for the transportation of groups. All drivers of minors for church events have undergone a criminal background check.
PERMISSION FOR TREATMENT AND DISCHARGE
My permission is granted for Abernathy United Church staff member or sponsor in charge of the trip or event to obtain necessary medical attention in case of sickness or injury for CHILDREN LISTED ABOVE. I/We, the undersigned, do hereby release, and forever discharge all staff, volunteers, and Abernathy United from any and all claims, demands, actions or cause of action, past, present, or future arising out of any damage or inquiry while participating in the event. We further accept financial and physical responsibility for the return of our child(ren), should the adult supervision find it necessary to send him/her/them home (as applicable).
I, the parent or legal guardian of the children listed above, grant Abernathy united church my permission to use the photographs for any
legal use, including but not limited to: publicity, copyright purposes, illustration, advertising, and web content.
Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such
use.
